The Minnesota HomeCare Association, Minnesota Hospital Association, and Stratis Health (Lake Superior QIN) is leading a community-based initiative to improve care coordination within and between care settings to reduce readmissions. An analysis on if patients receiving home health services have fewer readmissions revealed that:

Only 55% of Medicare fee-for-service beneficiaries with a hospital referral to home health actually received home health services.
Of those that received services within 5 days of hospital discharge, the 30-day readmission rate was 17.8%, while those that were referred to home health and didn't receive services, the rate was 27%.

Join us to help narrow this gap!

This webinar will provide information about the home health gap, and the new statewide virtual collaborative which will:

Identify the multitude of issues and barriers for receipt of home health services
Discover best practices at hospitals and home health agencies on collaborative approaches to increasing the acceptance and use of home health services following a hospitalization
Develop tools for spread throughout the state to increase the uptake of home health services and reduce readmissions

Participants will have the opportunity to join the collaborative at the end of the webinar.

Hospital discharge staff: discharge/care transition nurses, social workers
Home health agency staff
Others that are interested in learning about this collaborative opportunity, such as those working in nursing homes and assisted living facilities.

Discuss the analysis findings for post hospital discharge home health referrals in Minnesota
Identify the effects of the home health referral/uptake gap
Understand how to participate in a statewide virtual collaborative to reduce the home health gap.
