Quality Improvement Across the Continuum

2016 Long Term Services and Supports Quality Conference

Join home and community-based service providers and nursing facilities for the 2016 Quality Improvement Across the Continuum conference.

Online registration

The online registration website is open for individuals and groups to register for the conference.  Payment will be collected through an external online registration website.

 

The registration fee includes:

·         Admission to all conference sessions and materials

·         Morning and afternoon breaks, breakfast and luncheon provided during conference hours

·         Networking opportunities and more!

 

Present a poster- deadline for submissions is May 16th

The conference seeks to provide opportunities for attendees to learn about innovative quality improvement efforts in home and community-based services and nursing facilities.  The conference welcomes poster presentations from individuals and organizations on quality improvement projects and ideas.

 

Submit your poster session here today! Five randomly selected entries will have their conference registration waved. The last day to submit poster ideas is May 16th at 4:30pm.

 

Conference session information

More than 30 service providers from across Minnesota will share first-hand experiences and lessons learned from implementing quality improvement projects.  Participants will leave with strategies and practical ideas to apply to their own organizations.

 

Select project topics include:

·         Quality Improvement 101 and data development strategies

·         Adapting an evidence-based diabetes prevention program to better support Somali elders

·         Innovative partnerships with local colleges and businesses to improve peoples’ quality of life and other communication efforts

·         Integrating mental health and home care services to provide more holistic services for people with mental illness

·         Improving infection prevention and control and other wellness programs
