I’m sending this an update on the development of a common prior authorization form/ The members of MCHP had agreed to a common form and it was my understanding that this would be used by just the Council’s members. Kathryn Kmit from the
Council took it to the MDH Administrative Uniformity Committee (AUC) for approval, with a suggestion that it be used by all plans.
DHS and the county-based purchasing organizations were not involved in this development. Kathryn was asked to Chair a Technical Advisory Group to discuss the form and she kindly requested that I be named Co-Chair!
You will see in the email below that they are in the process of scheduling 1-2 meetings. I believe it will be important for at least one provider to be present for the review of this form – someone who is familiar with the various forms
and can speak to the challenges. Kevin has suggested that I take the approach of “it would be ideal to have one form, but if just the Council’s members adopt this form, it’s more manageable than it is today.” DHS is taking an early stance that all forms need
to be consistent…which they aren’t today.
I’m looking for a few volunteers to attend at least the first meeting with me. Please let me know ASAP if you or one of your staff (or another member you might suggest) can participate. Thanks!!

At the last Operations meeting, September 8, the AUC agreed to form a temporary TAG to review a recently developed “UNIVERSAL HEALTH PLAN/HOME HEALTH AGENCY AUTHORIZATION REQUEST FORM.” Kathryn Kmit of the Minnesota
Council of Health Plans and Kathy Messerli of the Minnesota HomeCare Association will co-chair the TAG. They are planning an initial and possible follow-up meeting of the TAG, and have requested that your response to the accompanying Doodle poll regarding
your availability for a TAG meeting on the dates and times listed. Two of the dates and times with the best response will be selected for the meetings. (If the work is completed after one meeting and the second meeting is not needed, the second meeting will
be canceled.) The TAG will be reviewing and discussing the universal PA form and accompanying instructions at the meeting, which can be found with the Sept. 8 meeting materials on the AUC website, at
