One of our tags will is the supervision in the personnel record of HHAs.  I asked how that is being done and Cindy the surveyor said that the agencies that are compliant have all the nurses carry a sheet of paper for each aide and then jot the supervision time and date and either initials or record # on each aide’s sheet and then turn it in every 3  months for the personnel file. 
The tag is 144A.4797 Sub. 4 which says” A home care provider must retain documentation of supervision activities in the personnel records.”  So Cindy said their trainers are telling the surveyors that it means All supervision when it doesn’t even say that. 
So that was my Monday story. 
I kept asking what is reasonable and useful way to comply and finally after re-reading their guidance and the 2nd surveyor looking at it also decided that we just need to put in our own policy how often we are doing direct supervision of the aide on a form that can go into the personnel file.  That was the close of the survey.  We still have the tag but at least it makes sense how to fix it.
We also got one on med reconciliation – that was a VA patient with a page and a half of meds so that figures.
