I as well am very interested and MHCA should be at the table.  We definitely need to have work done on Palliative Care from a home health perspective that may or may not have access to providers in the home in rural areas.  I may have a few leaders within our MBO that would be interested in assisting as well.    In case you are interested, this is the Seriously Ill Population program our ND agencies are applying for (must be a hospice but these folks can be dually served under HH if applicable).   This would be applicable to MHCA border agencies that serve both states.   BTW Greg, not in Wisconsin/SD.  Nebraska is the next closest.

Very interesting! I would definitely be interested in the Serious Illness Coalition, but my location might make participation prohibitive unless Zoom was an option. I am currently participating on the Rural Community-Based Palliative Care ECHO initiative with Stratis and it has been very interesting so I am looking forward to seeing what they do next.

 

On a side note, I think a MHCA Palliative Care team would be fantastic and Joelle would make the perfect chair! 😉

 

Lastly, I did not receive Kathy’s email just Andrea’s response. I search every folder to be sure. Hmmm…

I have a couple of updates that are timely so while we just met this week, I wanted to get this out now.

 

1)     Stratis Health and MN Network of Hospice and Palliative Care (MNHPC) are partnering to lead a MN based Serious Illness Coalition. They are inviting other organizations to join a launch committee and help shape the coalition’s work. The goal is to launch this effort at the MNHPC conference in April. I believe MHCA should be at the table and suggest we have a rep on the launch committee. Please let me know by early next week if you have a different opinion. Otherwise, I will move forward with that.

2)     The MN Leadership Council on Aging which MHCA is a member of hosts an annual Summit. This year’s (December 11) theme is on Age-Friendly MN – Engaging All Voices and Communities. I attend and encourage you to attend if this topic is of interest to you – or share it with others who may be interested. A flyer is attached. The registration cost is $75.
