Join MHCA and MNHPC for a comprehensive look at Palliative Care and Home Care.  With a variety of speakers and topics, this course will give you information on the building blocks of Palliative Care and different models that agencies have used.  This workshop also showcases a panel of providers and case studies, and will include information on Advance Care Planning.  Attendees will receive resources that will help your agency consider which model you want to implement!
