When I look at this I think about Hospice, Advanced directives, if you have a copy. Client and family requests.

What we do is put a notation on our Emerg Contingency Form if they have a POLST or Healthcare Directive. (and then try to obtain a copy) Our agency policy is still to have employees
call 911 if needed and let the EMS personnel know if there is any health care directives.
Not sure this meets the intent, so I’ll be interested to see what others do.

Is anyone willing to share what they include in their service agreements to meet the Minnesota licensure requirement for service plans noted below? I'm particularly interested in how to address the last item under
'd'
A contingency plan that includes:
1.
The action to be taken by the home care provider and by the client or client’s representative if the scheduled service cannot be provided;
2.
Information and method for client or client’s representative to contact the home care provider
3.
Names and contact information of persons the client wishes to have notified in an emergency or if there is a significant adverse change in the client’s condition, including identification of and information
as to who has authority to sign for the client in an emergency; and
4.
The circumstances in which emergency medical services are not to be summoned (consistent with chapters
145B and
145C) and declarations made by the client under those
chapters.
Our agency is looking at reformatting our service agreement and questions came up about the intent and what people list for this.
