Sorry – just returning from vacation and catching up. I think it would be really helpful to have a chart that spells out who can complete F2F, certify etc. so that providers
can quickly reference the chart rather than having to read the 2-page document (the chart wouldn’t replace the text, just supplement).
ED provider – no
Hospitalist during inpatient stay – yes
PCP – yes
SNF –
Surgeon –
Specialist in clinic –
Specialist during inpatient hospital -
Hospitalist during observation stay –
Etc….
