I’ve also attached a CTI Compliance Guide from NHPCO for reference, see pg 5.

 

The only way staff would ever ‘assist’ with a narrative statement is by sending clinical information (e.g. copies of records, copies of prognosis guidelines, etc) that the MD can review to help with writing their narrative.  Again, we don’t fill in anything on the CTI, this is all separate medical record documentation that is sent along with the CTI for the MD to complete.  We rarely need to do this, and never for our Hospice MDs as they have access to our records and review intake notes and assessments to find this data (weight, PPS, etc).  We only send this type of info if we have an attending MD who seems to be struggling with sending us a valid CTI for admission (e.g. they keep sending CTI back without narrative, or simply listing terminal dx, etc…without a narrative with clinical findings).

Hi all,

 

Quick question regarding the Hospice CTI. Does your staff (can they?) assist the provider in creating the CTI?  If your response is absolutely not, I would love to hear more about that. Our medical director is lazy and really wants nothing to do with it aside from signing a complete one. I’m a newbie to Hospice here and trying to learn the rules.
