Hi,
I am wondering if we can remove an ulcer/wound diagnoses at recert once the ulcer/wound is healed or do we need provider documentation in the medical record that it is “resolved”? We are no longer providing dressing
changes so it is not on our plan of care. I am under the impression that we would only include that in the diagnoses if we are addressing it in the plan of care.
If you do need provider documentation that the wound is healed/resolved, how are agencies getting that?
Here is the response from our coders : “There is no documentation from the MD that it resolved and last time he was seen for ulcer was on 2/22/23. I don’t know how I would query anyone since no MD has seen him lately.”
