Below is an old post from NAHC regarding standing orders, as well as an old CMS letter regarding this. Both are old references, but I believe still relevant to how the individualized plan of care is viewed today.
Accrediting bodies might have more specific language.
From NAHC, Regulatory Affairs (2009):
CMS does not permit standing orders for home health or hospice patients, but does allow protocols to be established and approved by the physician. Even when protocols
are implemented, the POC must still be individualized. I would caution against referring to these protocols as standing orders. I would also caution against sending physicians protocols on patients for which the protocol may not apply. For example, pain management
protocols where pain is not an issue.

I’m doing some research on standing orders and can’t seem to find any reference to being able to use them or not. My recollection is we can not use them but I can’t seem to find a regulatory statement anywhere to substantiate anything.
