Hi all!
We have our Compliance nurses review the therapy medications after each PT SOC. They look high level and follow up with staff and supervisor if they notice high risk issues, duplications or incorrect orders.
It is up to staff/ supervisor to complete the follow up with MD. We were just surveyed and they were satisfied with the process. I can share our homemade reference chart for what is within PT scope of practice and what is not if that would help too.

I’m not helpful because our therapists do not do SOCs. Does the therapist sign the 485? Who contacts the provider when discrepancies are discovered? This could be how you show that they are involved. I would hope that the surveyor just
asks about your process i.e. nurse reviews the list before it is sent to the provider and when you confidently have an answer to their questions, they accept it.
Curious what others do.

Hi everyone,
I wanted to tap into this teams knowledge to find out how other agencies are having the RN “review” medications when a therapist does the SOC? We have the nurse check to make sure the therapist has documented the
correct process for medication review (and look for documentation of follow up to MD if needed), but the nurse isn’t doing the complete review. Just wondering what other agencies are doing? Has anyone had issues with the surveyors with their process?
ยง484.55(c)(5) In rehabilitation therapy only cases, the patient’s therapist must submit a list of patient medications, which the therapist
must collect during the comprehensive assessment, to an HHA nurse for review. The HHA should contact the physician if indicated.
