Hi There – our clinicians are doing double work as well.  We used to complete paper documents and upload, but we struggled to get 100% compliance with uploading documents and the chase for them was too labor intensive.  We now have clinicians complete electronically which serves as our record and then clinicians complete the paper documents in the home.  We did explore the mailing piece but found that process to be ripe with risk and possible errors as well as costly. 

 

One other thing that we are doing that I don’t see mentioned is use of DocuSign.  We will utilize DocuSign with POAs.  It’s a multistep process but we’ve ironed out the kinks so it works pretty effectively.

That seems like a compliant plan to me; as long as the forms are being provided to the pt,  but I could not say for sure.  With our agency we can complete paper forms in the home and upload a photo of them to the pt chart.  This way pt has a copy to keep and we have or record of the paperwork electronically.

Hi! We’re trying to reduce duplication in work for our field clinicians. Currently, our process is to have clinicians fill out admission paperwork electronically for our records and then on paper for the client to keep in the home. We’re thinking about changing our process. We’d have the clinician fill out the paperwork electronically and then offer a copy to be emailed or printed and brought out at the next visit or the client could refuse a copy all together. This would be documented in the SOC documentation. Do you see any potential compliance issues with this process? Are you using a similar or different process that seems to go smoother?
