We have a Discharge Policy that includes criteria of “If patient or family/caregiver refuses or discontinues care”
I could argue that Missed Visits are a refusal of care. We do not have anything that specifically states ”if you miss xx number of visits, we will terminate services.”
I can think of an instance long ago that we wrote up a behavioral contract that included keeping visits as planned.
On the soft side, we use 3 Missed Visits as a gauge to start monitoring episode and start the conversations about consistent care needed for results.

visits do you allow your patients to make before terminating their services.
