I recently had a somewhat similar question and reached out to the Nurse consultant at MHCA. It was regarding homemaking only services for a client though. I would think that for a skilled service (nurse administering an injection) it WOULD be required.

Homemaker services do not require OASIS or a face-to-face so you should be okay to bill without the f2f visit.

Question

Hello! We have an interesting situation we would like your guidance on. We received a referral for a Medicaid client needing no skilled services only homemaking 1-2 times per week. We are having trouble getting him an appointment at the clinic for a face to face within 30 days of admission. We are reaching out to the clinic to explain the importance of a timely appointment. Then we got to wondering if the face to face is necessary since the client is not receiving a skilled service. We were told in our last survey with The Joint Commission in February of 2021 that clients without skilled needs do not need Oasis assessments and face to face assessments (this was discussed in the setting of private pay consumers though). Can you please provide some guidance on this situation? And what happens if we are unable to meet the 30 day after admission timeline? Thanks in advance! We so appreciate this service!

Is it my understanding that being a Medicare Certified agency that regardless of the patients that we accept, even when private pay, we are still required to have a PECOS certified practitioner as well as a F2F encounter? 

 

I had a strange call and the person continued to try and tell me that since it was private pay, they didn't need to send a face to face encounter? Is this maybe for non-medical home care agencies? They are wanting 4 injections to be given by a nurse and to pay privately for them, I am just confused now why a face to face or certifying MD wouldn't be required. 

 

I suppose I am just asking for confirmation to ensure I'm not misunderstanding something here.
