I’ll be interested in responses too.  We discharge from MC and open them under the new payer source.  Anytime there’s a change to or from MC we start a new episode.

We are wondering how other home care agencies handle this scenario:

Patient was admitted to home care with Medicare services and had straight Medicare, but halfway through the first cert period the patient was enrolled in a Managed Medicare/MA (dual prepaid plan) through their county.  Do you do a total discharge from agency for straight Medicare and then readmit under the Managed Medicare/MA plan or can you simply change payers?
