I based my answer on claims that I “pass-through” for snf patients who have Medicaid. These “room and board” claims have the snf facility ID in box 56. In this situation my hospice agency is identified by the
Tax ID in box 5 leaving the facility to be identified by the NPI in box 56.
For clinical service hospice claims where we that don’t require a separate facility identification I submit my agency’s NPI in box 56.

This is a hospice billing codes sheet from CGS which I’ve found very helpful:

For SERV FAC NPI, the CGS document is saying that it’s not put on the UB-04?? I know this field is on page 3 of the FISS/DDE claim form.
I looked at the Medicare billing manual, and it only talks about the institutional claim loop that this information goes into when you do an electronic claim [Loop
2310E] – doesn’t talk about a hard-copy claim.
Sorry, Marnie, that’s not a clear-cut answer.
