Is anyone getting their BCBS NOA's rejected with remark code M41 and N400?  I didn't think anything of it until the final claim was denied and it has the same remarks  as the NOA.  Now today I got another NOA rejected.

Yes, we are having trouble with Blue Plus MSHO (Medicare) and BCBS Medicare Advantage.

 

I had to call on a Blue Plus MSHO (Medicare) denial today, it denied for that very reason you mention.  I was told the claim did not have a NOA.  However, this was a subsequent claim and therefore does not need a NOA.  I had to give them the claim number from the NOA submitted at the time of admit and they were going to send it back.

 

I have had to call on multiple BCBS Medicare Advantage claims because they deny for no authorization.  However, we did get authorization from Evicore, and it is on our claim.  When I call they look it up and then send it back. 

 

The Blue plans have definitely been burdensome and have created extra work and a backlog of revenue.

Hello,

 

Are people getting paid for their BP MSHO claims?  I received payment for a few dates of service. The rest were denied for missing/incomplete/invalid value code or amount.  Would someone please advise what this means or what our claim is missing?

 

It has taken over six months to even get these claims to process.
