I would dc and do a new SOC when client gets discharged from the hospital.  You will have new F2F documentation and more than likely medical condition changes.

Subject: Medicare question

Hello Fellow Billers,

I am looking for some guidance.

We have a patient who has been on Medicare episode since 2.27.24.  So, we are now into our second 60 days which will end on 6.25.2024.  Patient was hospitalized on 6.5.24 and is going home tomorrow.  Are we better off discharging and readmitting?  Or do we resume with the expectation we will need another 60 days starting on 6.26.24?

Any help would be appreciated!

Thank you!
