I just had this issue come up just now, with a County we do not service very often. And thanks to this question and this fantastic group of people, I was able to guide the County worker in what she needed to do.

I have run into this many times. Once I explain they are usually good, but I have even had to help a CM complete the form.

Tell the County CM that they need to fill out a 5841 form and submit it to Ucare for them to generate an authorization for you. The 5841 form E-DOC area for them to find the form.

I have someone that has a CADI waiver, Ucare SNBC. We need an auth for SNV and HHA. Ucare is stating because there is a CADI waiver that we need to go to the county for the auth. The county is stating that they don’t auth the SNV and HHA. What do I do?
