Do they only want loc billed? Do you bill all days for perdiem on one line or separate?

If the patients VA is through MPLS, Melissa with MPLS VA, will need to get a referral from a VA provider to start the authorization process. I have the number if you need it. I also have a number for the VA in St. Cloud, same process. From:
