Traci, 

I am finally getting my respite paid using rev code 0580 and S5150. The description is required, that is the denial I had been getting once I figured out the code combination. My software has a claim edit that I can add that puts the description on the claim. So, I am no help as to how you or where it goes on the claim. It was a very frustrating and long process. Hopefully someone has additional info to give  you to be able to get your claims paid. Good luck!
