I have been attempting to bill an outpatient home care claim for a patient with Blue Plus of MN, it’s a PMAP MA plan and it keeps getting denied for invalid TOB? I typically bill a 329. I know MA has different bill types and I have tried those but nothing is working. Any ideas on where I can find a list of correct bill types for this type of plan? Or does anyone have any experience with this?

I feel kind of stupid for even having to ask this, but I get these so infrequently so I just don’t know where else to turn.
