I just wanted to make sure everyone is aware of the big change with 2024 Ucare authorization process for Ucare MHSO clients with a waiver CM.  According to the new authorization grid that was sent out MSHO members with a waiver are now required to have the waiver CM submit a 5841 form to Ucare for SNV/HHA visits.  I have always had to request a 5841 for the CONNECT/SNBC members with a waiver CM but never with MSHO members.  I have contacted Ucare regarding this new process and I am waiting for a response back.
