In the two different Home Health Agency I have worked for, the fee schedule was figured out using a lot of information. How much it costs to provide the service, how much does DHS reimburse, cost of living increases each year, how much can you negotiate with pay sources. And anything else that might be unique to your agency. When I started billing in 1997, BCBS was reimbursing $92 per nurse visit is has more than doubled since then.

I would like to inquire, is there a standardized fee schedule that is federal or statewide published for agencies to follow to create their fees for discipline services?
If so where is that located? Are there rules to change your current fees?

Do agencies set their own fee scales?
