Weird. As long as I have both the value codes 61 and 85, I have no issues at all. Value code 85 is FIPS state code, then the FIPS county code. Here is a link to the MN county codes.

Is it a date thing? Was the code good for one date span and not the other? That's the only thing I can think of besides Humana being difficult as per usual.

Good afternoon,
I have a Humana claim that denied for an invalid FIPS code. I don't see anything wrong with the code and have had the claim sent back for review a few times. This same patient had another claim
that generated the same FIPS code on a claim with different dates of service and that claim paid. Humana continues to maintain their decision even when they have reviewed this patient's other claim that paid. Is there a FIPS code lookup that I could possibly
use to send in proof to Humana that it's a valid FIPS code? Has anyone else had this issue with Humana? Thanks!
