I have had the same denials. I have had to change the initial admitting claim to TOB 342 and everything other claim I send is TOB 343. I have been getting paid and not having any other issues. I do not change TOB for a discharge.

I have another billing issue I wanted to run past you guys to see if anyone has had the same problem. Optum claims are being rejected in our clearinghouse (assurance) for discharge values invalid for final claim. I logged a ticket through assurance and they told me it's a system issue on Optum's side but no estimated time of resolution. Anyone else seeing this as well?
