I’m a hospice biller looking for prior auth information for a BCBS Anthem Indiana policy.  I have found nothing useful.

 

But I thought maybe the attached could help with home health inquiries.

I’ve recently tried obtaining auth from Carelon, and they denied it due to the services being provided in MN, and stated the auth must come from the local.  I then requested auth from the local BCBS and also got denied.  I’m now just planning on appealing the claims if denied, as I’m not sure how else to proceed.

Its in change healthcare vender. It’s a plan for Cleveland cliffs mining company of which we have tons of employees in my area. Cleveland Cliffs is based out of Pennsylvania so we get strange plans coming through for Minnesota norms… lol

Anyone have experience with this plan? We have gotten 2 of these in the last couple months and Found out they have a payer ID 34009, phone number 888 230 7338, home health needs auth from Carelon which is a third party contractor for claims and auths?..... Been jumping through hoops with them today.
