The part in red would not apply, since the 322 is a RAP and we no longer send those.
Does the service date with the Rev Code 0023 match up with the first date of service you are reporting on your clam?

I keep getting medicare claims returned for this reason: IF THE ADMISSION DATE OF THE 3X2 CLAIM IS EQUAL TO THE STATEMENT
FROM DATE, THEN THE EARLIEST 0023 REVENUE CODE LINE DATE SHOULD ALSO EQUAL -OR- REVENUE CODE 0023 WAS NOT FOUND ON THE CLAIM
-OR- EFFECTIVE ON OR AFTER 7/01/01 AND THE CLAIM IS A FINAL BILL (3X9) EACH 0023 REVENUE CODE SERVICE DATE MUST ALSO CONTAIN A VISIT SERVICE DATE FOR THE 0023REVENUE CODE. MAKE THE NECESSARY CORRECTIONS AND
RESUBMIT.
The problem I have is usually a final claim on a discharged patient. Has anyone else been having this issue? Did something change in how medicare wants their claims?
