I use the 1500 for our Ucare Homemaking.  I only have 4 people that receive HM and are on Ucare (thank goodness).  I have to manually add in Change Healthcare because EPIC and CHC don't seem to jive for the electronic claim to go.  I got tired of opening tickets and having no resolution.

For Home Health Billing on Homemaking charges/services, does anybody have any type of “cheatsheet”  on which payer(s) may require the CMS1500 form vs UB04 form?   In particular,  I am curious if you are using the 1500 form for UCare (on Homemaking).
