I just got a bunch of denials with this reason today. These are MA plans so I  believe there is a UCARE issue. I will call this afternoon and find out and give feed back if I get any.

I was looking back through the old listserv emails because I have a ucare denial for my January billing on a patient for missing/invalid/incomplete charge.  Am I to understand that Ucare is now PDGM billing?  I have a FFS claim that has already paid for the month of January and according to the provider bulletin, it was going to continue to reimburse according to the terms of their current provider agreement, so I don't understand how I could have one FFS claim paid and the other FFS denied for invalid.
