We have been contracted with Humana for over a decade and bar none they are the most difficult payer to work with.  Between their arbitrary denials and non-standard 'insert just about anything here' it's like pulling teeth to get them to pay for even the most simple services.

VA has required me to use modifiers  for the therapies in the home  - GP for Physical Therapy, GN for Speech, and GO for Occupational.

 

Although, Humana should be paying on a PPS episode and it would seem odd to add a modifier for intermittent visit(s) or particular service(s) which were not provided more than one time in a day…????
