Hi – when I file a PA with Kepro, I send in the 485 and the med list (because most med lists are too long to type all the meds in!).  We tend to get a fair amount of denials (or so it feels like we do) but then if answer the questions/provide the information they are looking for – i.e  - does the patient have family or friends that can assist them – they usually approve it. Occasionally I do send in the visit notes for supplemental documentation on why visits are needed, but I always start with just the 485 and med list.
