Here are just a few thoughts because I have experienced similar situations.

Are you entering it with the HIPPS code that Medicare processed it under? Our claims always generate the generic HIPPS code 1AA11. Make sure you run it through the pricer with the correct HIPPS code that should have been used not the generic HIPPS code to make sure the pricer shows the correct expected payment amount. 

Was there a penalty on your final claim? I am honestly not sure how you can tell on the remit or in fiss. I usually end up calling to ask. If I check in fiss and see that my NOA was received late, I check in our clearinghouse when it actually went through. Medicare had that system issue a while ago that the provider needed to add the facility zip code to the NOA and F9 back through. Well, it fixed the issue and allowed the NOA to go through but then updated the received date in fiss so we received late penalties on some of ours which is why the paid amount was different. I had to send in replacement claims with the KX modifier and remarks stating the NOA was received timely and I used the date our clearinghouse showed. That fixed the issue for me. 

Not sure if this helps at all. Your situation may be totally different, but these were just a few things I noticed when our expected payment didn't match what Medicare paid.

Haven't had that happen yet but today I got a final claim paid at 1/4 of what we expected.  2nd 30 day claim, not a LUPA and the final oasis sent and accepted prior to submitting claim.  The only odd thing  is Medicare changed the HCPCS to NNNNN.  Explanation is code 97 which makes no sense.

 

I called NGS and they elevated it to a Tier 2 which will re-run it through the pricer.  Now I wait to hear their explanation. 

 

Just wondering if anyone else is seeing this?

Hello,

 

Has anyone had their NOA deny with reason code: U537F ?  I see this is a known system issue with Medicare, when the CWF does not recognize the discharge status from the last home care stay.  I am wondering if anyone knows of a work around for this issue?  Maybe this was addressed in a past email, I apologize if I am repeating something.  There is no direction on NGS, they just say the proposed resolution/fix is TBD.
