I have had the same issue billing to Optum and I spoke to Tenisha and she stated that is a system issue with the 34_ TOB

This is a paper I got from the VA regarding hcpcs codes and rev codes. G0156 if for home health aide services and S5130 is for homemaker services. So for G0156 I use rev code 0571 and S5130 I use 0580. One thing I have been getting denials for is remark code 21953 which states the procedure/revenue code is inconsistent with the type of bill. Missing/incomplete/invalid revenue code. I get this when I send a TOB 322 so I change it to 323 and then it pays. Hope this helps
