Place of service codes are typically on a cms 1500.  UB04 would be like the revenue code.

Subject: [EXTERNAL] issues with Medica?

Is anyone having trouble getting paid from Medica.  They are randomly paying some of our Advantage plans, and then denying most of our claims.  I bill with Occurrence code 50, with HCPCS codes.  Now they are denying with CO96 remark codes and when I call, I’m told we need place of service codes….where does that go on a UB04?

If you have any tricks, I would really appreciate it.
