Humana told me to bill on a 1500 claim form and gave me instructions on how to bill. I followed their guidelines got paid and they took it back. When I asked why they now said they wanted it submitted on UB 04, didn’t get paid so I feel like no matter what you do you might not get paid.

I think they want HIPPS code on claim even though its per visit. We are having trouble with them also but not sure how.

Subject: Humana help
We rarely have Humana replacement claims and I am struggling to get some skilled nursing and PT/OT claims paid. The first time I sent out the claims they denied for type of bill, evidently got that fixed because now they're erroring for place of service.
I'm trying to bill fee for service, not pdgm. I'm using G codes. We use nightingale notes and I have the claim template set up as institutional with admission date and auth required.
Anyone that can help me?
