I have this problem when the county has not sent in the "form" they are supposed to or mysteriously didn't receive it.  

I just had one on monday, the county had sent in the form but mispelled the persons name and BCBS rejected it a week ago by sending her an email  and told her it went back all the way to November when she had initially it in. 

BCBS had paid Nov & Dec but starting denying in January and I just was able to get a person to tell me what was going on with it when Feb & March were denied.

Subject: [EXTERNAL] Blue Plus denials- no auth T1031
 
Hey everyone, 
I am getting denials from Blue Plus due to it stating that there is no auth for code T1031, however the representatives I have spoken to at BCBS state that is odd since it states it is denied, but also that it doesn't require prior authorization. So they have taken a list of several patients that were denied and will be creating a 'project' on this issue, but I'm wondering if anyone else is having this issue?
