What would you say about this?

 

60 day cert period is from 11/4/2020-1/2/2021

The 1st 30 day period RAP and Final have been sent

The 2nd 30 day period I will send the RAP soon. Since the 2nd 30 day final will fall in 2021, how will that work? If I resend an additional RAP on Jan 1st, 2020 for the 2nd 30 day payment, it will be outside of the 5 day window. Will this still meet the new requirements, if I submit this final in 2021, following current procedures. Or how would you proceed?

This is my understanding:

If a 60 day cert period is from, let’s say: 12/20/2020 – 02/17/2021

The RAP for 12/20/2020 will be like we do now.

The RAP for 01/18/2021 will be the No Pay RAP.

Is this if the second RAP begins anytime in 2021?

 

For instance, let’s say the second payment period begins 12/27/2020. A portion of the second payment period would still fall into 2021, but it would have started in 2020. Would we still submit the RAP in 2020 and another No pay rap in 2021, all for the second payment period?

If the 60 day episode spans from 2020 – 2021 and the second 30 day period begins in 2021, you will need to submit a No Pay RAP for the second 30 day period that begins in 2021.

“S” Suspended does not necessarily mean it was accepted, it is in a review, there is the potential for it to Reject.  But most likely will be accepted if there are no mistakes.

I use Ability but if it is in there as “S” suspended does that mean same as accepted?  Usually either “S” or RTP(return to provider) but RTP can take couple of days…..

I think it’s going to be a struggle at first, we are going to use that Grouper to get the HIPPS number code as quickly as possible , then we were thinking of putting it directly into DDE versus the clearing house, (like we do with the Hospice NOE) we do not have the billing package in Carefacts so I hand enter everything anyway.

I am the only one here too at this point who sends the RAP’s.  I am not sure how it will work, I definitely plan on training some others to do it too, as soon as we see how this all flows in our system.

So we are using Axxess for our EMR and they have made some changes for us to complete submissions of both RAPs within the 5 day window. We are aggressively working on getting paperwork from clinicians completed more timely and our goal is within that first 24 hours. We have outsourced our coding that guarantees coding and OASIS review within 24 hours as well (not that OASIS is needed, but coding will be), then we QA it as soon as that process is complete to submit for the RAP. We are hopeful, not so confident at this point, that all of this will be completed within that 5 day window and accepted. We are still unclear how this will play out over weekends and holidays since we are a small agency, I guess that means I will be working 7 days a week? Still trying to figure this process out to ensure timely filing. Any other smaller agencies out there have a plan for timely submission and acceptance of RAP over weekends or holiday periods? Cross train more of our staff/clinicians to be able to complete some of this process?

First – the generic HIPPS code can be any one of the HIPPS codes? No. For carefacts, they are building the one in.  They also will have the claim have the generic hipps code attached if it was on the rap. But Medicare has the occurrence code pointing to the oasis to match up to the claims for pricing and carefacts will have pricing for the claim built from actual hipps code.

 

Second – the RAP and the Final need to have the same HIPPS code, if we use a generic HIPPS code, it is ok if it is different from the HH Grouper HIPPS code that Medicare assigns to the Final claim? Rap and claim have to have same hipps code and it is fine because they do pricing off oasis hipps

 

Third – Does the diagnosis have to be acceptable with the generic HIPPS code used?  No diagnoses does not have to. I believe medicare is giving the generic hipps code to be used by everyone… will get clarification

Yes. It sounds like carefacts is going to build so we can bill both 1st and 2nd  30 day RAPs with the generic hipps code so we can if we need to and when the claims get billed, they will bill also with the generic hipps code but carefacts to generate revenue based on actually hipps code. SO minus a few kinks we will run into with the software glitches, I think we are going to be really well set. Our goal is to use the generic as little as possible but we know the reality of how easy that would be to do… lol…
