Is county not willing to change your service agreement?

I am noticing that we have denied claims coming from DHS due to not having the correct HCPCS code on the claim. After researching further our EMR has a HCPCS code T01031 for "LPN Visit" but our SA is for a code T01030 only. Our EMR has under this code as a "Skilled Nurse Visit."

So my question would be, can a LPN or RN complete the visit under the HCPCS code of T01030?

If not, are all the visits completed by an LPN then unbillable?
