Some of our Tri West authorizations have ended and the patient is back to VA for SN and Therapy. And yes, most of our patients are with the St Cloud VA. While they were under Tri West, we bill the RAP 322 and
the Final 329.
The St Cloud VA has just added the PT visits to the SN authorization on our patient so the two disciplines are billed together.

Marnie, aren’t you billing SNV and Therapy to Triwest? Or are these St. Cloud clients? If billing Triwest the type of bill needs to be 322 (RAP) and 329 (FINAL).

Melissa, I did call the help desk and they were truly denying, I was using 322 and 329. They couldn’t tell me “how to bill”, but stated a lot of claims she sees are using 323 so I guess I will go with that and

Recently I have had new patients whose claims I bill as a 322 and even a discharged patient that I billed as a 324 deny for TOB. So I had to resubmitted them as a 323.

Hello, have you called the community care center to find out if the claim really should have been denied? I have had many claims denied and found that when I have called the person helping me will attach notes to the claim and let the processors
know to review the claim again because the claim should have been paid and not denied.
I personally saw this as a denial on some of mine as well but have not called yet. I am going to try to do that this week to see if they have an answer for me. I use TOB: 329.
